This is the *SONY OEM Power Adapter for the PS1 Slim *for the US and this
eBay auction I found at least for the US includes free shipping. Starting
at $12.99 USD for one (more than one discounts) They currently have listed
more than 10 available.

Not affiliated with them but did a search in case I was in the market for
one myself.

Below I've cobbled a short list of everything you mentioned.
